Best I can do is. If you go to Bing images and fill in the what box with "Allis Chalmers All-crop dual wheels " I found pictures that could show you the location and approximate spacing. keep clicking on pics and I found a combine with 3 different angles.

next best guess is type in "Allcropharvester" .com, that is the man that has a lot of experience building them. Tom Yasnowski is his name.

Tandem axle mounts in front of the standard axle. Not the same as duals.
I found an illustration in the parts book for the 72 combine, page 165, on the allcropharvester.com. The dual wheel setup is also shown.

The left side tandem wheel mounts to the rear of the standard axle, not the front.

The only down side with this design is that you must remove the axle assy. to repair or change the tire.

I mounted the tandems on the All-Crop that has the two row corn head. So the up side to this design is that the left side tandem wheel runs in the center of the row. The Allis or Common design would put the wheel right on the row.

With the corn head it may be different, but what I like about having tandems with a grain head is the tongue floats off the ground, making hook up and manual rolling of the combine easier. 

I have access to two parts machines with the regular Allis tandems, but they are about 110 miles from home, without wheels, with the axles half buried, and big trees through them. Not sure if it's worth it to dig them out to remove the tandem attachments or not.
