I have a D21 that I need the instruction plate that goes behind the key switch. If anyone has one or knows where I may get one, please contact me or reply on this post.

Steiner's carries them.  Some of the vendors here might as well.

that acs354 isnt the correct switch plate for a 21, a 21 uses the d series switch and the others that this fits uses the indak style,
But as far as I am aware this is the only one that is available and you could use it if you cut the hole out in this switch plate as it will be hidden behind the escutchin or however you spell it,  will be hidden by the chrome nut dealee that holds the key switch solid
